Документ, определяющий распределение обязанностей среди исследователей на месте проведения исследования и содержащий образцы оригинальных подписей исполнителей
В документе указаны исполнители и их обязанности в связи с проведением клинического исследования на данной клинической базе, внесены фамилии и инициалы всех лиц, вовлеченных в клиническое исследование, которые имеют право на получение информированного согласия от пациента, а также право на заполнение ИРФ и внесение в них исправлений. Каждый из перечисленных исполнителей должен поставить свою оригинальную подпись, после чего документ заверяется оригинальной подписью ответственного исследователя.
